Parents/Guardians,

Physicals must be dated June 1, 2022, or after, to be valid for the 2022-2023

school year.

**Log onto FormReleaf.com

**If this is the first sport the athlete is participating in at the HS:

Please "Sign Up" by creating a profile then Log In.  Go to "Find Organization" and type in North Penn. Click on "North Penn” then scroll down to “Programs” and click on “2023 Spring Athletics”. Please choose a sport and scroll down to “Participant Information” and fill in the info for the student athlete for whom you are registering. Once you have done that, “Submit” will appear at the bottom and after you click that, your student athlete will be registered for their winter sport. To confirm the registration, you will receive a confirmation email.

Please print out Section 6: "Health History" and Section 7: "PIAA Comprehensive Initial Pre-Participation Physical Evaluation & Certificate of Authorized Medical Examiner". Complete Section 6 and take along with Section 7 to an Authorized Medical Examiner. Once Section 7 has been completed by the Authorized Medical Examiner, return Section 7 to the NPHS Athletic Office or NPHS Head Athletic Trainer, Melissa Rosenberger. (Form must be completed after June 1, 2022ocean).

**If the athlete already registered during the fall/winter season:

To confirm the registration, you will receive a confirmation email.

Spring Sports: Impact Testing

Every athlete that plays an impact sport (all sports except track and tennis) must take the ImPACT test every 2 years, before their season begins. The test is taken at home, at your convenience and should not be done after a vigorous workout or on a phone. It should be done in a quiet area and it will take about 30-45 minutes.

To access the ImPACT test, complete the online registration mentioned above. Once that registration is completed, you will receive an email from ImPACT within 72 hours. The email will be sent to the email address used to complete the online registration only if you need one. If you took an ImPACT test last year, you will not need to take another one and will NOT get a link emailed.

If you have questions regarding this process, please contact Melissa Rosenberger, North Penn Athletic Trainer at [email protected]

Physical, online registration and impact testing are all due on February 27th, 2023 for all sports.  These dates are to ensure paperwork is filled out properly and everything is turned in before preseason starts. If anything is missing, the athlete will not be allowed to participate on the first day!
